Abstract
This chapter examines the wellbeing of elderly people and caregivers within professional and family caregiving processes in Italy, against the backdrop of rapid population ageing and the growing demand for long-term care. Drawing on qualitative research conducted by the University of Palermo research unit, the study analyzes ten interviews with paid caregivers and ten dyadic interviews involving paid caregivers and reference persons (family caregivers or care managers). The findings highlight the relational complexity of home-based care, structured around a triangular configuration: older person, paid caregiver, and family caregiver. Although paid care is formally regulated by contract, relationships frequently evolve into para-familial bonds characterized by trust, reciprocity, and emotional exchange. Social capital emerges as a crucial resource sustaining these care arrangements and mitigating caregiver burden. However, both formal and informal caregivers experience significant emotional and psychological strain, often insufficiently recognized by institutions. The study underscores the need for policy interventions that strengthen care alliances, enhance service accessibility and proximity, and promote community-based models capable of supporting the wellbeing of the entire caregiving system.
