Abstract
In selected cases, surgical treatment for patients with chronic constipation can be successful. However, knowledge of the various surgical techniques and their advantages and disadvantages is a prerequisite for tailoring the best procedure for the individual patient. The most important issue for functional interventions is the right indication. It is very important to give the patient realistic expectations. Pain and flatulence cannot be improved by an operation, only the transit time can be shortened. An accurate diagnosis and assessment of the quality of life are prerequisites for a successful operation and a satisfied patient. Only if conservative measures fail should surgical intervention be considered. For example, if the colon transit study determines a diffuse distribution of markers, this study should be repeated before performing an operation. Otherwise, a failure rate of approximately 30% can be expected. Patients with chronic constipation often also have psychiatric disorders. Therefore, physiological investigation and a preoperative psychiatric evaluation are essential for carrying out a coordinated and patient-specific therapy. In principle, ODS is usually significantly more likely than STC19,20.
